If you are a new patient to Smile Studio 87, we want you to be prepared for your first visit with Dr. Sumayyah Khan or Dr. Maja Sukovic. If you have not already filled out our patient forms prior to coming in, we encourage you to come in early to do so. We also ask that you bring in:
- A form of ID
- Your dental insurance card (if applicable)
- A list of any medications you may be taking.
If you require antibiotics before your dental treatment, please take them as prescribed. Contact your doctor before your visit if you are not sure whether you need antibiotics.
